Appointment Guidelines

 

You can schedule an appointment to see Dr Bubra by calling our clinic at (02) 8031 7733.

Our friendly team will send you a registration form before your initial appointment. When you arrive for your appointment, please ensure you have the following items:

  • A referral letter from your doctor. Please note that referrals from General Practitioners are valid for 12 months from the date of issue, whereas referrals from Specialists are valid for 3 months.
  • Details of your private health insurance.
  • If applicable, your Medicare card and Department of Veterans Affairs (DVA) card.
  • Any X-rays, MRI, CT scans, or other relevant investigations and reports. While many imaging investigations may be accessible online, please verify beforehand that we can access these images before your appointment.

Additionally, for patients covered under workers’ compensation or CTP;

  • Please provide your claim number.
  • Include the name and contact details of your claim officer.
  • Supply details of your insurer.
  • Specify the date of injury.

Dr Bubra diligently gathers spinal outcome measures for every patient under his care. This practice serves two essential purposes: monitoring the progress of your spinal treatment and contributing valuable data to enhance the care provided to other patients. You may be requested to complete forms related to spinal outcome measures during each visit to Dr Bubra’s clinic.

While he endeavours to maintain timely appointments, occasional delays may arise due to the complexity of cases or unforeseen surgical emergencies.
